PLEASE READ THE FOLLOWING INSTRUCTIONS VERY CAREFULLY!
Attorney Sam Malat and I are collecting information to compile
a comprehensive database of victims of insurance crime.  This database will
be VERY USEFUL in terms of 1) legal action; 2) criminal action; 3) being
able to get a handle on the numbers of victims and the patterns of the crimes.  At the present time, because of the problems with the court system,
and the overpowering financial and legal resources of the insurance
companies, it has been extremely difficult, time consuming and expensive
trying to force the insurers to give us the raw data or statistics.
(It seems that memory loss is a requirement for employment at these companies.  When you read the depositions of their employees, you will note
that they have very poor memories.  The companies also practice willful
incompetence, willful stupidity and willful blindness and don't keep
or their legal representatives don't seem to know if they keep statistics on
claim denials, confidentiality agreements and patterns of denial).

At the present time we are interested in those having problems getting
their DISABILITY CLAIMS PAID, by any carrier.

If you currently have an attorney, I would suggest you fill out the form,
have your attorney check it, and send it in as soon as possible.  If
you don't have an attorney, we still need your data.

WARNING:  Your cooperation will make or break our ability to compile this
database.  The database will make it easier for ANYONE who has been screwed
out of disability payments or subjected to harassment, etc. to get justice.
If you don't cooperate, we can't HELP YOU.

A WORD FOR THOSE WITH CONFIDENTIALITY AGREEMENTS - READ WHAT YOUR AGREEMENT
SAYS.  You may only be prohibited from stating the AMOUNT OF THE SETTLEMENT.
You may still be able to answer most of the other questions.  We need
ANY INFORMATION YOU CAN SUPPLY.  If you can't provide information because 
of the settlement, just give your name, etc. and fill in the other blanks
with the word "GAGGED."

Here are the ground rules.  Sam and I are going to distribute this form.
In the near future I will be distributing blank affidavit forms, one for
attorneys handling these cases and one for claimants, since many of you
don't have attorneys.  We expect your cooperation with us.  We expect
to get fairly complete data on AT LEAST 50 CASES WITHIN 30 DAYS!! I'm
serious about this.  If this occurs, we will distribute what information
we have collected at slightly above cost (postage, copying, and maybe a
little something if it involves a  significant amount of time) to anyone
who submits a questionnaire and claimant/attorney affidavits to us.  Those
who do not help us will not
be eligible to receive this information.  If we get a good response we will
provide our services and copies of this data and copies of the affidavits
as cheaply as possible as long as our money holds out.

AFTER 30 DAYS, if we have not received information on 50 CLAIMANTS, our
patience will have reached its limit.  If we don't get significant
cooperation from victims and attorneys within the first 30 days, there will
be a cost (a significant one) for the database and affidavits.  Sam and I
don't mind doing the work, but FIGHTING YOUR FEAR really takes it out of
us.  I will sell or distribute, AT MY DISCRETION, to claimants or others
that I feel have a need for this information (i.e. hopefully the
Department of Justice).
 
Your names and other information WILL NOT BE POSTED ON THE INTERNET.  That is 
not the purpose here.  We need the information so that we can compile the
statistics, present them to the courts, and if the insurer doesn't produce
data that can counter it, we will have compelling evidence of a pattern.
They can't produce the data to counter the pattern because the pattern
exists.   So if you want the information, for litigation, to ensure
that your claim will get paid, continue to be paid, you must:

1) FILL OUT A QUESTIONNAIRE FOR YOURSELF AND SEND IT BY E-MAIL ASAP
2) SEND IN A SIGNED AND NOTARIZED AFFIDAVIT on YOURSELF BY SNAIL MAIL ASAP
3) TELL YOUR ATTORNEY TO SEND IN HIS/HER AFFIDAVIT BY SNAIL MAIL ASAP
4) CONTACT AS MANY OTHER CLAIMANTS/ATTORNEYS as YOU CAN WHO MAY NOT BE
ON THIS LIST AND DISTRIBUTE THE FORMS TO THEM AND URGE THEM TO GET THE
INFORMATION TO US.

The complete instructions for each question will be available AFTER the
list of questions or in the next Coollist post.  You may want to print
out these postings and READ EACH INSTRUCTION and then refer to the
instructions while you CAREFULLY ANSWER EACH QUESTION.

Final warning:  Please do not call me on the phone to tell me your
excuses for why you can't or won't fill out the forms or submit
affidavits.  I don't want to hear it.  Either do it or don't do it.
If you won't help us, then I wish you the best of luck, because you

Specific Instructions for Database will follow the list of fields below.
Because of the way the Coollist is set up you may only be able to print part
of the message at a time.  Helpful suggestions:

1) get someone to help you who knows how to use computers to:
2) highlight the entire post (that means running your cursor over the
message until it turns blue).  Then go up to the top of your screen and
click the "Edit" box.  A list will drop down.  Click the "copy" button.
Everything you highlighted (everything on the blue background will be 
copied).  Now minimize your Internet window by clicking on the little
"dash" in the right upper corner.  Then open your "Word" or "Microsoft
Word" program.  I do this by clicking the "Start" button in the lower
left corner, then "Programs," then "Office 97," and then "Microsoft Word."
That should give you a nice white sheet with your cursor in the left upper
corner.  Now go back to "Edit" and click it.  When the list comes down,
click "Paste."

All the material you copied from the Coollist post will magically appear
on your Word document.  Then hit "File."  When the window comes down,
hit "Save."  In the box that appears you can title this DATABASE.

3) Now you have the database and instructions easily available to Print
out on your copier.  Or you can copy it, fill it out, etc.
